Garrod defendió con vehemencia la individualidad química de la enfermedad. Con el conocimiento disponible hoy, después del proyecto del genoma humano es cada vez más evidente que aun gemelos idénticos con la misma enfermedad pueden presentar características muy distintas en cuanto a su severidad y pronóstico ( 47, 48).

Además, observó agudamente que algunas de estas enfermedades podían ser de aparición tardía. En este aspecto también se anticipó varias décadas; sabemos hoy que muchas de las enfermedades de difícil diagnóstico en el adulto son o parecen corresponder a EIM sin diagnosticar.

Ciento diez años después de que Garrod propusiera por primera vez en Londres el concepto de errores innatos del metabolismo, podemos afirmar que se anticipó e indicó la ruta que se debe seguir para descubrir y diagnosticar un grupo de enfermedades cuyo estudio ha hecho y sigue haciendo aportes incalculables a las ciencias biológicas y a la medicina. Garrod nos enseñó que somos únicos y que por tanto los médicos y la medicina nos deben tratar como individuos, no como un miembro más de la especie humana.

En el capítulo 13de este libro se cotejan algunas de las principales tesis de Garrod con lo que conocemos actualmente sobre la comprensión, diagnóstico y manejo de estas enfermedades, campos en los cuales se han hecho enormes avances, pero cuyo desafío principal, la cura permanente, sigue sin lograrse. Ya en 1914, Garrod, hablando del efecto de la dieta libre de purinas para la gota, recordaba a su audiencia que con ello no se estaba tratando la enfermedad sino procurando modificar los síntomas. Mucho se ha logrado con los tratamientos nutricionales, la terapia de reemplazo enzimático, la reducción de sustrato y las chaperonas farmacológicas, que sirven para detener los síntomas, prolongar la vida y dar un mejor nivel a los pacientes con errores innatos del metabolismo, pero su afirmación de 1914, en esencia, sigue siendo válida.
